The PESCI (Pre-Employment Structured Clinical Interview) is a structured assessment designed to evaluate the readiness, clinical competence, and professional suitability of international medical graduates (IMGs) seeking registration to practise medicine in Australia. It assesses a candidate’s medical knowledge, communication skills, ethical decision-making, and understanding of the Australian healthcare context through scenario-based and structured interview questions.
Unlike traditional multiple-choice tests, PESCI uses structured scenario-based interviews with open-ended clinical and situational questions, conducted under strict time constraints. This format is designed to challenge candidates to think critically and communicate their reasoning, clinical judgement, and ethical decision-making skills in real time, simulating the demands of actual medical practice in Australia.
PESCI scenarios may include role plays with simulated patients, case-based discussions, and interactive handover exercises, each typically lasting about 15 minutes. These components require candidates to demonstrate advanced diagnostic reasoning, professional communication, and cultural competence, with expectations appropriate to supervised roles rather than specialist or fully independent GP practice.
Questions are developed to specifically assess suitability for the proposed practice position and context in Australia, with panels of experienced clinicians conducting the assessment to ensure fairness and thorough evaluation. The standard is set higher than for resident trainees but below the level required for Fellowship, focusing on readiness to practise safely in a supervised capacity.
